Letter From the Nurse's Office

Dear Parent/Guardian:

Health and safety is our first priority for all of our scholars.  To ensure we are supporting their needs, please review and complete the following:

·Please obtain an updated copy of a physical with immunizations.

·Please keep in mind all medications must be delivered by an adult to an adult.

·All medications need to have the original pharmacy label.

·All prescription medications require a signed Doctor’s Order and a Parental Consent form.

·EpiPen (epinephrine auto injectors) require a doctor’s order and a Food, Allergy, & Anaphylaxis Emergency Care Plan. Please print and attach your child's photo, along with parent/guardian signature, and return to the School Nurse on the first day of school. These forms must be renewed as needed and updated at the beginning of each academic year.

·Inhalers require a Doctor’s order and an Asthma Action Plan. Please print and attach your child's photo, along with parent/guardian signature, and return to the School Nurse on the first day of school.  These forms must be renewed as needed and updated at the beginning of each academic year.

Parents are responsible for completing and providing all the health paperwork, Immunization records, and up-to-date physicals before a child can be allowed to attend school.

All students that are in need of taking medication, must provide proper documentation with the medication to the School Nurse, who will administer the medication at the prescribed time.
