At Our Lady of the Assumption School, the health, safety, and well-being of our students are among our highest priorities. Our Nurse's Office provides a safe, supportive environment where students receive care and attention throughout the school day, allowing them to stay healthy, focused, and ready to learn.
OLA is fortunate to have two dedicated registered nurses, Nurse Lauren and Nurse Paula, who work collaboratively to support the medical needs of our students. Whether addressing everyday illnesses, managing chronic health conditions, or responding to injuries, our nurses are committed to helping every child thrive.
A Healthy School Environment
To help protect all students and staff, OLA maintains a nut-free school environment. We ask all families to carefully review lunch, snack, and classroom items to ensure they do not contain nuts or tree nut-based products.
Peanuts, tree nuts, and products containing nuts are not permitted at school under any circumstances. We ask all families to carefully read ingredient labels before sending food items to school.
Please do not send items that:
To help ensure student safety, homemade baked goods are not permitted for classroom celebrations or shared events.
Supporting Student Health
To ensure the safety of all students, families are required to provide updated health information and medical documentation as needed. This allows our nursing team to appropriately support students during the school day, school-sponsored activities, and athletic programs.
Parents are encouraged to keep the Nurse's Office informed of any changes to their child's health status, medications, allergies, or emergency contact information throughout the school year.

Required Health Documentation

- An updated physical examination form from child’s physician
- Current immunization records
- Medical clearance for participation in school activities and athletics (when applicable)

Medication Procedures
For the safety of our students, all medications administered during school hours must follow school health policies and Massachusetts state regulations.
Important Medication Guidelines
- All medications must be delivered by an adult directly to either nurse.
- Medications and required forms may be dropped off at Meet & Greet or on the first day of school at the Nurse's Table located at the Main Entrance until 8:30 AM.
- A recent photograph of your child should be attached to all medication authorization forms.
- All medications must be in their original container with the original pharmacy label attached.
Prescription Medications
All prescription medications require:
- A signed physician's order
- A completed Medication Administration Consent Form signed by a parent or guardian
EpiPens (Epinephrine Auto-Injectors)
Students requiring an EpiPen must provide:
- A signed physician's order
- A completed Food Allergy & EpiPen Authorization Form
Inhalers & Asthma Medications
Students with asthma who require medication during the school day must provide:
- A signed physician's order
- A completed Asthma Action Plan
