Please email your child will be absent before the start of the school day [email protected]
in addition to the notice of illness, please include their first and last name, and homeroom-
example: Jane Doe, 1A D'Avolio will be absent due to _______________.
Parents are responsible for completing and providing all the health paperwork, Immunization records, and up-to-date physicals before a child can be allowed to attend school.
All students that are in need of taking medication, must provide proper documentation with the medication to the School Nurse, who will administer the medication at the prescribed time.
Please remember that students should remain home for 24 hours after fever!

ALLERGY AWARENESS
Our Lady of the Assumption School is proud to be an 'Allergy Aware' School.
Peanut products and any other products containing tree nuts are NOT allowed in school under any circumstances due to severe allergies experienced by many of our students. This includes all school-sanctioned events during and outside of school hours.
As manufacturers are always changing their labeling, parents are encouraged to read product labels before sending food items into school for lunch or snack. No homemade baked goods are allowed.
Avoid labels that list nuts/tree nuts in ingredients or say “May contain traces of peanuts/nuts” or “Manufactured in a facility that also processes peanuts (and/or other nuts).”

LETTER FROM THE NURSE'S OFFICE

Health and safety are our first priorities for all of our scholars. To ensure we are supporting their needs, please review and complete the following:
·Please obtain an updated copy of a Physical with Immunizations and Medical Clearance to participate in school activities and sports
·Please keep in mind all medications must be delivered by an adult to an adult.
·You may drop off medications and forms on the first day of school or at meet ·and greet to the Nurse's Table at the Main Entrance until 8:30am.
In addition to the form, please attach your child's photo, sign, and return to the School Nurse on the first day of school.
All medications need to have the Original Pharmacy Label
All Prescription Medications require a signed Doctors Order and Medical Delivery Parental Consent Form (link below).
EpiPen (Epinephrine Auto_Injectors) require a signed Doctors Order and complete the Epi Pen Food and Allergy Form(link below).
Inhalers require a Doctors Order and an Asthma Action Plan (link below).
