Helping Us Keep Every Student Safe and Healthy
At Our Lady of the Assumption School, student health and safety are a shared partnership between families, healthcare providers, and our nursing team. To ensure we can properly support students during the school day, we ask families to submit all required health forms and medical documentation in a timely manner.
The forms below help our nurses provide safe care for students with medical conditions, allergies, asthma, and medication needs while at school and during school-sponsored activities.
If you have questions about any form or are unsure which documentation is required for your child, please contact the Nurse's Office. We are happy to assist you.
Required Health Forms
Medication Authorization & Delivery Form
This form must be completed whenever a student requires medication to be administered during the school day. Massachusetts regulations require written parent permission and healthcare provider authorization for prescription medications administered at school.

Food Allergy & EpiPen Action Plan
Students with food allergies or those requiring an EpiPen must have an up-to-date Allergy Action Plan completed by their healthcare provider. The plan provides important emergency instructions and helps our staff respond quickly and appropriately if an allergic reaction occurs.

Asthma Action Plan
Students with asthma who may require inhalers or other asthma-related medications during the school day must have a completed Asthma Action Plan on file. This document outlines symptoms, medications, emergency procedures, and healthcare provider instructions.

Medication Drop-Off Procedures
To ensure student safety, all medications must:
Be delivered by an adult directly to either nurse.
Remain in the original pharmacy-labeled container.
Include all required physician orders and parent consent forms.
Be accompanied by a recent photo of the student when requested.
Medications may be dropped off during Meet & Greet or on the first day of school at the Nurse's Table located at the Main Entrance until 8:30 AM.

- Medications administered during school hours

- Asthma diagnoses
- Inhaler use during school hours
- Participation in athletics and physical activities when asthma management is needed

- Current Immunization Records
- Medical Clearance for School Activities and Athletics (when applicable)
