Letter From the Nurse's Office

Dear Parent/Guardian:

Health and safety are our first priorities for all of our scholars. To ensure we are supporting their needs, please review and complete the following:

·Please obtain an updated copy of a Physical with Immunizations and Medical Clearance to participate in school activities and sports

·Please keep in mind all medications must be delivered by an adult to an adult.

·You may drop off medications and forms on the first day of school or at meet ·and greet to the Nurse's Table at the Main Entrance until 8:30am.

·All medications need to have the Original Pharmacy Label

·All Prescription Medications require a signed ***Doctors Order ***and Parental

·Consent*

·EpiPen (Epinephrine Auto_Injectors) require a signed ***Doctors Order ***and a

·Food Allergy & Anaphylaxis Emergency Care Plan. Please attach your child's photo, sign, and return to the School Nurse on the first day of school.

* These forms must be renewed as needed and at the beginning of each academic year.

Inhalers require a Doctors Order and an Asthma Action Plan. Please attach your child's photo, sign, and return to the School Nurse on the first day of school.*These forms must be renewed as needed and at the beginning of each academic year.

Please do not hesitate to contact the Nurses office at 781-599-4422 x2 inform me of any pertinent information regarding your child's medical history and/or specific needs.

Parents are responsible for completing and providing all the health paperwork, Immunization records, and up-to-date physicals before a child can be allowed to attend school.

All students that are in need of taking medication, must provide proper documentation with the medication to the School Nurse, who will administer the medication at the prescribed time.

Please remember that students should remain home for 24 hours after fever!
